The Adams Center for Mind and Body is committed to your health and well-being. We are working diligently to respond to all inquires in a timely manner and your call is important to us.

The Adams Center is currently operating on a waiting list basis.  We recommend that you look for other Providers as well.

If you are interested in being put on the Wait List for your Intake Evaluation with Clinical Director, Sue Adams, please call us at 802-859-1577, extension 400 and leave a message. Please indicate that you would like to be put on the Waiting List and include some times we might reach you by phone. We will do our best to call you back as soon as possible to set up an appointment.

If you are already on the waiting list, we are working diligently to schedule your Intake Evaluation with Clinical Director, Sue Adams, as quickly as we can.

We are accepting new clients for complementary therapies such as Reiki and bodywork.  If you are interested in any of our Group Therapy or complementary therapy options, please call us at 802-859-1577, extension 400 with some best times that we might reach you.  We will return your call and schedule you for those services.

INTAKE EVALUATION

Your care begins with an assessment appointment with our Clinical Director, Sue Adams, to address treatment issues unique to you.  Together, you and Sue will co-create a plan utilizing traditional, evidenced based therapies, as well as complementary therapies, such as energy work and body work.

Sue works with your primary care physician, as well as the providers at The Adams Center, to coordinate services.  Every treatment plan is uniquely customized based on background and experience, as well as your individual needs.  All of the professionals at our Center are dedicated to supporting you in your recovery.

TO SCHEDULE AN INTAKE EVALUATION:

1. Please call 802-859-1577, extension 400, to reach Stephanie, Front Desk and Admissions Coordinator or fill out our Confidential Inquiry Form below to send a private message to Admissions.  (To protect your privacy, it is important not use the internet to communicate personal data or private medical information.)

2. If you call by phone, please be prepared with you insurance card in hand.  We will check your benefits with your insurance company for the services offered here and have that information prepared at the time of your assessment appointment with Sue Adams.

There is a one time $45.00 registration fee that is payable by cash, check, or credit card, due at the time of your Intake Evaluation with Clinical Director, Sue Adams, M.A., Licensed Psychologist-Master.  (This fee is in addition to the telehealth charge for your intake evaluation, which will be billed to your insurance company.)

This appointment is to coordinate the best plan for treatment, as well as determine which provider within our group or outside in the community would work best with you.  In some cases other therapeutic services may be recommended and incorporated into your customized treatment plan.

GOOD FAITH ESTIMATE Notice

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services, including psychotherapy services.

Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
Make sure to save a copy or picture of your Good Faith Estimate.
